摘要:
家庭医生是我国居民健康的“守门人”，当前我国家庭医生签约服务模式主要有：杭州模式、上海模式、盐城模式、厦门模式、定远模式。本文以彩虹模型为理论框架，从宏观、中观、微观及支持要素四个层次以及系统、专业、组织、临床、功能及规范六个维度，对五种模式进行对比分析，为我国推进家庭医生签约服务提供建议。
Abstract:
Family doctors are the “gatekeepers”of the health of Chinese residents. At present， the family doctors contract service models in China mainly include：Hangzhou mode，Shanghai mode，Yancheng mode，Xiamen mode，and Dingyuan mode. This paper took the rainbow model as the theoretical framework，from the macro，meso，micro and support factors and the six dimensions of system，profession，organization，clinical，functional and norm，compared and analyzed the five models to promote family doctors in China.
