摘要:
目的：借鉴RBRVS，评估中医医师工作量相对价值，试图让中医医师的劳动价值逐渐回归合理水平。方法：通过文献回顾、德尔菲专家咨询法构建针灸医师工作强度评价指标体系，使用问卷调查法开展基于针灸医疗服务项目的医师工作强度评价；选择某三甲中医院针灸科全体医师为观察对象，分别对医师亲自操作的10项针灸医疗服务项目的工作时间进行测量；利用RBRVS原理测算10项针灸医疗服务项目中医医师工作量的相对价值,采用Spearman的秩相关分析法，与广东省广州市针灸医疗服务项目收费价格进行相关性分析。结果：构建了医师工作强度评价指标体系，并确定了各指标的权重；测算了30名医师在10个针灸医疗服务项目上的工作强度相对价值、平均工作时间以及工作量相对价值，结果显示，项目之间内部定价存在不合理现象。结论：研究结果能客观反映医师在提供不同针灸医疗服务项目时的劳动价值，具有客观、可靠和较好的评价功能，能为政府完善针灸医疗服务项目内部比价体系提供合理建议。
Abstract:
Objective Learning from RBRVS,this paper aims to evaluate the relative value of TCM doctors' workload, and try to make the labor value of TCM doctors return to a reasonable level gradually.Methods The literature review and Delphi expert consultation method were used to construct the evaluation index system of work intensity of acupuncture doctors,and the questionnaire survey method was used to evaluate the work intensity of doctors based on acupuncture medical service items.All the doctors in the acupuncture department of a 3A TCM hospital were selected as the observation object and the working hours of 10 acupuncture medical service items personally operated by doctors were measured.The relative value of the workload of TCM doctors in 10 acupuncture medical service items was calculated by RBRVS principle,and the Spearman rank correlation analysis was used to analyze the correlation with the charging price of acupuncture medical service items in Guangzhou City,Guangdong Province.Results This paper constructs the evaluation index system of doctors' work intensity,determines the weight of each index, and calculates the relative value of work intensity,average working time and workload of 30 doctors in 10 acupuncture medical service items. The results show that the internal pricing between items is unreasonable.Conclusion The results of the study can objectively reflect the labor value of doctors when providing different acupuncture medical service items.The results of the study also have an objective,reliable and better evaluation function and can provide reasonable suggestions for the government to improve the internal price comparison system of acupuncture medical service items.
