摘要:
人口老龄化背景下，老年人的照护需求日益复杂多样，亟需通过整合，提高长期护理服务的供给效率。通过总结南昌市社区长期护理服务资源整合的实践，从宏观层面的系统整合、中观层面的组织整合、微观层面的临床整合三个方面，提出促进社区长期护理服务资源整合的具体建议：政府应瞄准自身定位，消除服务提供者之间的合作障碍，建立服务转介机制，完善社区个案管理工作。
Abstract:
Under the background of an aging population, the nursing needs of elderly people are increasingly complex and diverse, so there is desiderate to improve the supply efficiency of long-term nursing services through integration. By summarizing the integration practice of community long-term nursing service resources in Nanchang, and offer suggestions from macro-level system integration, meso-level organizational integration and micro-level clinical integration. These suggestions focus on promoting the integration of community long-term nursing service resources: the government should aim at its own orientation, eliminate the obstacles of cooperation among service providers, establish a service referral mechanism, and improve community case management.
