摘要:
“主动健康”是“健康中国”行动的重要举措，是健康医学未来发展的一种新模式。将“主动健康”理念融入社区慢性病管理，在政府引导和医院参与的基础上，社区、家庭和个人多方协作，建立支持性的健康维护环境，通过引导积极的生活方式等干预措施，使居民由“被动健康”到“主动健康”，从而获得保持健康和预防疾病的能力，真正实现慢性病防治关口的前移，维护人民健康。
Abstract:
"Active health" is an important measure of "Healthy China" Action and a new model for the future development of health medicine. It is necessary to integrate the concept of "active health" into community chronic disease management, and on the basis of government guidance and hospital participation, cooperate communities, families and individuals together to establish a supportive health maintenance environment. Through intervention measures such as promoting an active lifestyle, residents can change from "passive health" to "active health", so as to gain the ability to maintain health and prevent diseases, and truly realize the advancement of prevention and treatment for chronic diseases and safeguard people's health.
