目的：探讨药品集中采购政策对医院抗菌药物使用的影响，为医院更好地平衡集中采购与抗菌药物管理之间的关系提出建议。方法：收集山东省某医院2016—2021年抗菌药物使用数据，采用A.M指数法进行分析。结果：近五年，山东省某医院的门急诊抗菌药物使用率为4.02%，住院患者使用率为41.20%，抗菌药物使用强度为42.35，抗菌药物费用占比为4.08%。集中采购政策实施后，医院抗菌药物的用量因素指数呈上升趋势，价格因素指数平稳略降，结构因素指数下降，有低价药取代高价药的趋势，但药品费用变化不大。结论：集中采购政策可以有效控制抗菌药品价格，但仍需配套相应的管理措施才能促使临床合理用药，降低药品费用。
Abstract:
Objective This paper explored the influence of the centralized purchase policy on the use of antibiotics in a hospital in Shandong Province, in order to provide suggestions for the hospital to better balance the relationship between the centralized purchase and antimicrobial management. Methods The data of antimicrobial used in a hospital from 2016 to 2021 were analyzed by A.M index analysis. Results In recent five years, the utilization rate of outpatient and emergency antibiotics in the hospital in Shandong province was 4.02%; the utilization rate of inpatient antibiotics was 41.20%; the intensity of use of antibiotics was 42.35; and the proportion of antibiotics expenses was 4.08%. After the implementation of the centralized purchase policy, the dosage factor index of antibiotics in hospitals showed an upward trend. The price factor index decreased steadily and slightly, and the structural factor index decreased expressing that had a trend that low-price drugs replaced high-price drugs. However,the drug cost changed little. Conclusions The centralized purchase policy can effectively control the price of antimicrobial drugs, but hospitals must take corresponding management measures to promote rational clinical drug use and reduce drug costs.
