目的：基于合作博弈夏普利值法，探究构建医共体利益分配方案。方法：运用焦点小组访谈法，总结医共体交易成本与收益特点，构建医共体经济效果模型，利用合作博弈夏普利值法，计算医共体利益分配方案，并选取宁波某区医共体成立前后运营数据进行测算。结果：医共体组建后所有成员单位均实现收益正增长，整体收益增加了42.5%。基于夏普利值的利益分配方案下，牵头医院可分得65.3%的增益，每家成员单位可分得3.5%~11.9%的增益。结论：基于夏普利值的医共体利益分配方案能客观准确地反映参与机构的贡献程度，有助于提高利益再分配的公平性和激励性，可作为多元法人医共体内部利益再分配的依据。
Abstract:
Objective To establish a benefit distribution within the county medical community based on cooperative game theory and Shapley value method. Methods Focus group interviews were conducted in July and August 2020, based on which the characteristics of transaction cost and benefit of the medical community were summarized and the economic effect model of the medical community was constructed. The cooperative game theory and the Shapley value method was used to calculate the benefit distribution of the medical community, and empirical research was conducted on a medical community in a district in Ningbo. Results All member units had achieved income growth after establishment of the medical community. The overall benefits increased by 42.5%. According to the benefit distribution based on the Shapley value, the leading hospital could receive 65.3% of the increased benefits, and the member units could receive 3.5% to 11.9%. Conclusions The benefit distribution based on Shapley value is fair and incentive, which can reflect the member units' contribution more objectively and accurately. The Shapley value method can be used to determine the redistribution of profits within the medical community.
