目的：以县级公立医院为例，探索资源自主配置、运营管理能力对医院成本效率的作用机制。方法：以重庆市76家县级公立医院为样本，采用TCF-SFA模型测算医院成本效率，采用面板Tobit回归模型分析资源自主配置、运营管理能力对医院成本效率的作用机制。结果：2012—2018年样本医院成本效率为0.8125，并以1.08%的增速逐年提高；人员规模自主程度等变量与医院成本效率显著正相关，床位自主程度等变量与医院成本效率显著负相关。结论：资源自主配置、运营管理能力对公立医院成本效率的作用机制存在异质性，亟需通过强化医院成本核算能力、夯实成本核算结果运用机制等措施，提升公立医院成本效率。
Abstract:
Objective This study took county-level public hospitals as example, aiming to explore the role mechanism of resource autonomy allocation and operation management capability on the cost efficiency. Methods A sample of 76 county-level public hospitals in Chongqing was employed to measure their cost efficiency by TCF-SFA model.A panel Tobit regression model was used to analyze the mechanisms of the role of autonomous resource allocation and operational management capacity on hospital cost efficiency. Results The cost efficiency of the sample hospitals was 0.8125 from 2012 to 2018 and increased at a rate of 1.08% year by year. The degree of autonomy of staff size was significantly positively correlated with hospital cost efficiency, while the degree of autonomy of bed capacity was significantly negatively correlated with hospital cost efficiency. Conclusion There is heterogeneity in the mechanisms of the effects of autonomous resource allocation and operational management capacity on cost efficiency in public hospitals. There is an urgent need to enhance cost efficiency in public hospitals through measures such as strengthening hospital cost accounting capacity and consolidating the mechanism for applying cost accounting results.
