目的:探讨户籍身份对健康教育服务利用的影响机制。方法:利用“中国流动人口动态监测数据”,基于户籍身份的双重属性,将居民分为本地非农户籍人口、本地农业户籍人口、外来非农户籍人口和外来农业户籍人口四类,比较其健康教育服务利用的差异,并探讨户籍身份影响健康教育服务利用的作用机制。结果:居民在健康教育服务利用上存在显著的户籍差异;社会保障、社会参与、信息获取是户籍身份影响健康教育服务利用的主要机制。结论:提升流动人口的服务利用水平,应成为各地推动基本公共卫生服务均等化的重点。
Abstract:
Objective To explore the unequal utilization of health education services among the residents with different household registration status in China, and to promote the equalization of basic public health services. Methods Using the "China Migrant Population Dynamic Monitoring Data (CMDS)" in 2017, the study divided the residents into four categories based on the dual attributes of household registration status, namely, the local non-agricultural residents, the local agricultural residents, the migrant non-agricultural residents and the migrant agricultural residents. The study tested whether there were significant differences in the utilization of health education services among groups with different household registration identities, and explored the mechanism by which household registration status affects the utilization of health education services. Results There were significant differences in the utilization of health education services among the four groups of people, and there were also "internal-external" differences caused by the place of household registration and "urban-rural" differences caused by the nature of household registration. Mechanism analysis showed that social security, social participation, and information acquisition were three important channels through which household registration status affected the level of health education utilization. Conclusion The household registration status has caused the residents' unequal utilization of health education services, and this inequality is more reflected in the "inside-outside" division brought about by the property of the household registration.
