摘要:
目的：分析我国31个省（区、市）基层医疗卫生资源配置效率及其影响因素现状。方法：利用三阶段DEA模型对2020年数据进行静态分析，利用Malmquist指数对2016—2020年的数据进行动态分析。结果：目前我国基层医疗卫生资源配置效率总体较差，主要受技术进步制约；在剔除环境变量和随机干扰的影响后，2020年我国基层医疗卫生资源配置综合效率均值为0.702，5个省（区、市）的综合效率等于1；环境变量对资源配置效率影响显著，其中地区生产总值和人均可支配收入对资源配置效率提升有积极作用，政府卫生支出和65岁及以上人口占比对资源配置效率提升有负向影响；2016—2020年基层医疗卫生资源配置全要素生产率总体呈下降趋势。结论：政府应结合当地实际情况合理配置医疗卫生资源，加强基层医疗卫生机构网络体系建设；基层医疗卫生机构应重视人才队伍建设，并全面落实老年人服务优待政策。
Abstract:
Objective To analyze the allocation efficiency and influencing factors of primary medical and health resources in 31 provinces in China. Methods The three-stage DEA model was used for static analysis of 2020 data, and the Malmquist index was used for dynamic analysis of 2016-2020 data. Results At present, the overall allocation efficiency of primary medical and health resources in China is relatively low, mainly constrained by the level of technological development. After excluding environmental variables and random interference, the average comprehensive allocation efficiency of primary medical and health resources in China in 2020 was 0.702, and the comprehensive efficiency of five provinces was 1. Environmental variables have a significant impact on the allocation efficiency of primary medical and health resources, of which gross regional product and per capita disposable income have a positive impact on the allocation efficiency, and government health expenditure and the proportion of people over 65 have a negative impact on the allocation efficiency. From 2016 to 2020, the total factor productivity of the allocation of primary medical and health resources showed a general downward trend. Conclusion The government should allocate medical and health resources reasonably based on local conditions, and strengthen the network system construction of primary medical and health institutions. Primary medical and health institutions should attach importance to the construction of health talent teams and comprehensively implement preferential policies for elderly services.
