目的：分析高血压患者社区机构就诊连续性及其对医疗费用的影响。方法：基于H市医保结算数据，采用服务连续性指数（COCI）测算高血压患者社区机构就诊连续性，采用固定效应模型分析就诊连续性对医疗费用的影响。结果：2016—2018年高血压患者社区机构就诊连续性指数（COCI）均在0.9以上，患者年龄、性别、是否签约家庭医生是社区机构就诊连续性的影响因素；社区机构就诊连续性有利于降低患者医疗费用。结论：推进家庭医生签约服务，以高血压等慢性病管理为切入点，落实基层首诊、双向转诊制度，提升社区机构就诊连续性，减轻患者就医负担。
Abstract:
Objective To analyze the level of continuity of care at community health centers for patients with hypertension and its impact on health care costs. Methods Based on medical insurance settlement data from outpatient clinics and pharmacies in H city, the continuity of care index(COCI) was used to measure the level of continuity of community visits for hypertensive patients, and a fixed-effects model was used to analyze the impact of care continuity on annual outpatient visits and total annual outpatient drug costs for patients with hypertension. Results The mean of community COCI index for hypertensive patients was above 0.9 in all years from 2016 to 2018. Patient age, gender, and whether to sign a family doctor were the influencing factors for the continuity of care at community health centers. The continuity of care at community health centers was beneficial for reducing health care costs of patients. Conclusion It is necessary to promote the signing services of family doctors, implement first-visit and two-way referral system at primary care institutions with the management of chronic diseases such as hypertension as the starting point, improve the continuity of care at community health centers, and reduce health care burden of patients.
