目的：了解甘肃省两县区老年人卫生服务利用现状及其影响因素，为完善医疗卫生服务体系建设，提高老年人卫生服务可及性提供参考。方法：构建安德森卫生服务利用行为模型，分析老年人卫生服务利用的影响因素。结果：样本地区老年人近一个月门诊就诊率为4.03%，近一年住院服务利用率为12.98%；多因素Logistics回归分析显示，以前吸烟、到达最近医疗机构时间大于30分钟的老年人门诊服务利用率较高；患慢性病、政府救助对象、离家最近的医疗机构为县级及以上医院的老年人住院服务利用率较高；需要因素对老年人卫生服务利用的影响最大。结论：样本地区老年人卫生服务利用率较低，应通过加强基层卫生服务体系建设、完善医疗保险救助制度、落实基本公共卫生服务项目，提高老年人卫生服务利用，防止因贫致病、因病返贫。
Abstract:
Objective To understand the current situation and influencing factors of health service utilization for the elderly in two counties in Gansu Province, and to provide reference for improving the construction of medical and health service system and the accessibility of health services for the elderly. Methods Anderson behavioral model of health service utilization was constructed to analyze the influencing factors of health service utilization for the elderly. Results The consultation rate of the elderly in the sample areas in the past month was 4.03%, and the utilization rate of inpatient services in the past year was 12.98%. Multifactorial logistics regression analysis showed that the utilization rate of outpatient services was higher for the elderly who had smoked before and whose arrival time to the nearest medical institution was more than 30 minutes. The utilization rate of inpatient services was higher for those who suffered from chronic diseases, those who were recipients of governmental assistance, and those whose nearest medical institution to their home was a county-level or higher hospital. The need factor had the greatest influence on the utilization of health services for the elderly. Conclusion The utilization rate of health services for the elderly in the sample areas is low. It is necessary to improve the utilization of health services for the elderly by strengthening the construction of the primary health service system, improving the medical insurance assistance system, and implementing the basic public health service projects, so as to prevent diseases caused by poverty and returning to poverty due to diseases.
