目的:总结浙江省医疗服务价格改革现状,为开展国家医疗服务价格改革省级试点扩面提供参考。方法:从项目设置、项目价格差异、项目新增与调整机制等方面,分析浙江省医疗服务价格特点及影响。结果:杭州等3个地市的医疗服务价格与省级价格一致;温州等5个地市自主定价项目与省级一致,但基本医疗服务项目不完全相同;宁波等3个地市的基本医疗服务项目价格与自主定价范围均与省级不完全相同;近年来,浙江省主要基于专项政策进行医疗服务价格项目调整。结论:浙江省医疗服务价格项目设置的省市一致性较好,项目价格的省市差异性较大,一二类手术价格存在省市“倒挂”,三四类手术比价相对合理,常态化医疗服务价格管理机制尚未建立;建立方法科学、环节清晰的医疗服务价格管理机制,适度放开特需项目管理权限,试点先行、小步快走,推动地市医疗服务价格改革落地。
Abstract:
Objective To summarize the current situation of medical service price reform in Zhejiang Province, and to provide reference for carrying out the provincial pilot expansion of national medical service price reform. Methods From the aspects of project setting, project price difference, project addition and adjustment mechanism, the characteristics and impact of medical service price in Zhejiang Province were analyzed. Results The prices of medical services in three municipalities including Hangzhou were consistent with the provincial prices. The autonomous pricing projects in five municipalities including Wenzhou were consistent with the provincial level, but the basic medical service projects were not identical. The prices of basic medical service projects and the scope of autonomous pricing in three municipalities including Ningbo were not exactly the same as the provincial level. In recent years, Zhejiang Province mainly adjusted medical service price projects based on special policies. Conclusion The consistency of the project setting for medical service prices in Zhejiang Province is good between province and municipalities, and the difference of project prices is large between province and municipalities. The prices of the first and second types of surgery are "inverted" between province and municipalities, and the price comparison of the third and fourth types of surgery is relatively reasonable. A normalized medical service price management mechanism has not yet been established. It is suggested that Zhejiang Province should establish a medical service price management mechanism with scientific methodology and clear links, moderately release the management authority of special medical need projects, pilot first with small steps and moving quickly, and promote the implementation of the reform of municipal medical service prices.
