目的:分析2017—2021年我国全科医生配置公平性的区域差异以及动态演进情况,为实现全科医生均衡配置提供参考依据。方法:采用卫生资源密度指数评价全科医生配置公平性,采用Dagum基尼系数和核密度估计法分析全科医生资源配置公平性的区域差异及动态演进趋势。结果:2017—2021年我国全科医生卫生资源密度指数从0.123上升至0.190,全科医生配置公平性逐年提高;区域差异分析表明,组间差异对全科医生配置公平性差异的影响最大,其中东-西部地区差异是主要来源;动态演进研究表明,东部地区各省(区、市)全科医生配置公平性差异逐渐缩小,中部、西部地区各省(区、市)全科医生配置差异有不同程度的扩大趋势。结论:我国全科医生配置公平性逐步提高但绝对差异扩大,政府应强化宏观调控,加强顶层设计,完善全科医生资源供给机制,构建全科医生资源跨区域协同发展新格局。
Abstract:
Objective To analyze the regional differences and the dynamic evolution of the equity of general practitioner allocation in China from 2017 to 2021, and to provide a reference basis for realizing the balanced allocation of general practitioners. Methods The health resource density index was used to evaluate the equity of general practitioner allocation, and the Dagum Gini coefficient and kernel density estimation method were used to analyze the regional differences and he dynamic evolution trend of the equity of general practitioner resource allocation. Results The health resource density index of general practitioners increased from 0.123 to 0.190 from 2017 to 2021, and the equity of general practitioner resource allocation improved year by year. The analysis of regional differences showed that intergroup differences had the greatest impact on the differences in the equity of general practitioner allocation, with the differences in the east-west region being the main source. The dynamic evolution study showed that the equity difference of general practitioner allocation in provinces (municipalities and autonomous regions) in the eastern region was gradually narrowing, and the equity difference of general practitioner allocation in provinces (municipalities and autonomous regions) in the central and the western region had a tendency of expanding to varying degrees. Conclusion The equity of general practitioner allocation in China has gradually improved, but the absolute difference has widened. The government should strengthen the macro-control, enhance the top-level design, improve the mechanism of the supply of general practitioner resources, and build a new pattern of cross-regional synergistic development of general practitioner resources.
