摘要:
目的：分析我国县域慢病管理中心建设政策与地方实践进展，为推进县域慢病管理中心建设提供参考。方法：通过文献研究和案例分析进行政策分析和典型经验总结，利用立意抽样调查12个省份45个县级医院慢病管理中心建设情况，利用统计描述方法进行定量分析。结果：我国出台县域慢病管理中心建设政策以来，各地进行了积极探索，但机构名称、标准与规范尚不统一；县域慢病管理中心在协调县级医院相关科室方面正在发挥积极作用，但指导帮扶基层和上下协同工作有待加强，信息化互联互通水平有待提高。结论：借鉴典型地区经验，完善县域医共体医保支付政策、绩效考核与激励机制，提升信息化水平，进一步推进县域慢病管理中心建设，将其作为统筹县域内外资源、防控重大慢性病的节点和枢纽。
Abstract:
Objective To analyze the progress of policy and local practice in the construction of chronic disease management centers at county level in China. Methods Literature research and case analysis were used for policy analysis and typical experience summary. The construction of chronic disease centers in 45 county-level hospitals in 12 provinces was investigated by means of purposive sampling, and quantitative analysis was carried out by statistical description method. Results Since the construction of county chronic disease management centers proposed by the "The Project for One Thousand Counties" in 2021, active exploration has been carried out in various places, but the names, standards and norms of the institutions were not unified. The established county chronic disease management centers were playing a positive role in coordinating the relevant departments of county-level hospitals, however, they still needed to be strengthened in guiding and helping the primary health and coordinating the relevant work, and the level of information interconnection needed to be improved. Conclusion It is suggested to learn from the experience of typical regions, improve the payment policy, performance appraisal and incentive mechanism and information technology of county-level medical communities, and promote the construction of county chronic disease management centers. The county chronic disease management center should serve as a node and hub for coordinating resources inside and outside the county to prevent and control major chronic diseases.
