目的：构建住院医师规范化培训（住培）质量保障模型，为提升住培质量提供参考。方法：采用半结构化访谈法，对28名住培相关利益者进行访谈，基于扎根理论和SPO模型，进行三级编码，构建住培质量保障模型。结果：通过三级编码提取43个初始概念，构建住培质量保障体系模型，涵盖4个结构要素、4个过程要素和3个结果要素，并明确了各要素之间及其内部逻辑关系。结论：为提升住培质量，结构维度应聚焦培训基地质量管理职责、信息化建设与制度设计；过程维度强调思想建设、临床实践机会保障和激励机制的有效应用；结果维度通过多源评估反馈，提升住院医师的满意度与培训质量。
Abstract:
Objective To construct a quality assurance model for standardized residency training, and to provide reference for improving the quality of residency training. Methods Using semi-structured interview methods, 28 stakeholders related to residency training were interviewed. Based on the grounded theory and SPO model, three-level coding was performed to construct a quality assurance model for residency training. Results 43 initial concepts were extracted through three-level coding to construct a quality assurance system model for residency training, including 4 structural elements, 4 process elements, and 3 outcome elements. The logical relationships among the elements and within them were clarified. Conclusion In order to improve the quality of residency training, the structural dimension should focus on the quality management responsibilities of training bases, informatization construction, and system design. The process dimension should emphasize the construction of ideology, the guarantee of clinical practice opportunities, and the effective application of incentive mechanisms. The outcome dimension should improve the satisfaction of residents and the quality of training through the feedback of multi-source assessment.
