目的：分析社区康养模式的整合机制与存在问题，提出优化建议。方法：基于整合照料理论，提出社区康养模式整合框架，分析四个典型社区的康养模式存在的问题。结果：连接型、纵合型、协作型、完全整合型社区康养模式的特征分别为低协同低整合、低协同高整合、高协同低整合、高协同高整合，四种模式在支持体系、资源共享、机构运营、老年保障等方面存在一些问题，亟需优化改进。结论：通过加强社区康养服务保障机制、建立社区康养服务治理体系、健全社区康养服务工作网络等措施，进一步优化社区康养服务模式。
Abstract:
Objective To analyze the integration mechanism and problems of community health and elderly care model, and to put forward optimization suggestions. Methods Based on the integrated care theory, the integration framework of community health and elderly care model was proposed, and the problems of health and elderly care model in four typical communities were analyzed. Results The characteristics of connected, vertical, collaborative, and fully integrated community health and elderly care models were low collaboration and low integration, low collaboration and high integration, high collaboration and low integration, and high collaboration and high integration, respectively. The problems in the four models in terms of the support system, resource sharing, institutional operation, and elderly security urgently needed to be optimized and improved. Conclusion It is necessary to strengthen the security mechanism for community health and elderly care services, establish the governance system for community health and elderly care services, and improve the network for community health and elderly care services, so as to further optimize the community health and elderly care service model.
