目的：探讨医院床位配置的合理性，实现病床的精细化管理。方法：基于病例组合指数（CMI），校正医院床位周转次数测算方式，提出床位利用修正模型；构建床位效用指数（BUI）测算理论床位数，并与床位工作效率、调整床位工作效率、BCI和病床产出效率等评估方法进行比较。结果：病床周转次数与CMI呈明显的负相关关系，通过床位利用修正模型校正，将样本医院科室划分为效率型、周转型、压床型、空置型四类；对比分析显示，基于BUI的评价结果与科室属性一致。结论：床位利用修正模型和基于BUI的床位评估模型兼具客观性和科学性，有助于医院正确评价床位产出价值，合理解决床位冗余和闲置问题；激励表现欠佳的科室不断提升诊疗水平，向效率型科室转型，进而向“院中院科室”发展，助力医院进入良性运营轨道。
Abstract:
Objective To explore the rationality of hospital bed allocation, and to achieve refined management of hospital beds. Methods Based on the case mix index(CMI), the calculation method for hospital bed turnover rates was adjusted, and bed utilization correction model was proposed. The bed utilization index(BUI) was constructed to calculate the theoretical number of beds, and was compared with other evaluation methods such as bed work efficiency, adjusted bed work efficiency, BCI, and bed output efficiency. Results Bed turnover rates showed a significant negative correlation with CMI. By using the bed utilization correction model, the departments in sample hospital were divided into four categories of efficiency type, turnover type, retention type, and vacancy type. Comparative analysis showed that the evaluation results based on BUI were consistent with department attributes. Conclusion The bed utilization adjustment model and the bed assessment model based on BUI are both objective and scientific, aiding hospitals in accurately evaluating bed output value and solving the problems of redundant and idle beds. It is necessary to motivate departments with poor performance to continuously improve their diagnostic and treatment levels, transform into efficient departments, and further develop into " departments of hospital-within-hospital ", so as to help hospitals enter a healthy operating track.
