摘要:
人口老龄化的加剧以及疾病谱的转变，催生了大量的居家社区养老服务需求，亟须基于健康管理视角构建居家社区养老服务模式。居家社区养老服务模式下，健康管理服务流程包括健康检测、健康评估、健康干预和健康跟踪，同时配套信息交换、服务供给、服务反馈、动态监测等运行机制，从而实现老年人健康需求的精准供给。在健康老龄化发展战略指引下，应强化健康管理意识、整合养老服务资源、搭建信息共享平台、完善健康保险政策，促进居家社区养老服务模式的高质量发展。
Abstract:
The intensification of population aging and the shift of disease spectrum have generated substantial demand for home and community care services for older adults, calling for the establishment of a service model based on health management perspective. Under this model, the health management process includes health monitoring, health assessment, health intervention, and health follow-up, supported by operational mechanisms such as information exchange, service delivery, service feedback, and dynamic monitoring, thus achieving the precise supply of the health needs of the older adults. Guided by the strategy of healthy aging, it is essential to raise awareness of health management, integrate elderly care resources, build information-sharing platforms, and improve health insurance policies to foster the high-quality development of the home and community care model.
