摘要:
当前，基层慢性病防治面临多重挑战，县域医共体成为慢性病“防—治—管”一体化治理的理想载体。针对慢性病预防普及的“最后一公里”难题、治疗服务的同质化困局、管理效能的碎片化短板、“防—治—管”协同的机制性壁垒，县域医共体应从筑牢基层预防网底、统一规范诊疗标准、贯通县域管理流程、实现“防—治—管”联动协作四个方面入手，提升慢性病“防—治—管”一体化治理效能，推动健康中国战略在县域层面的精准实施。
Abstract:
At present, the prevention and treatment of chronic diseases face multiple challenges at primary level. The county-level medical community has become an ideal carrier for integrated governance of chronic diseases through "prevention, treatment, and management". In response to the "last mile" challenge of chronic disease prevention popularization, the homogenization dilemma of treatment services, the fragmentation weakness of management efficiency, and the mechanistic barriers of "prevention, treatment, and management" collaboration, county-level medical communities should focus on four key areas including strengthening primary prevention networks, standardizing diagnosis and treatment protocols, streamlining county-level management processes, and achieving the linkage and collaboration across "prevention, treatment, and management". These measures will enhance the effectiveness of integrated governance of chronic diseases through "prevention, treatment, and management"， and promote the precise implementation of the Healthy China strategy at the county-level.
