摘要:
当前，我国基本医疗保障制度在渐进改革的过程中仍存在横向不公平问题，不同人群、不同区域的筹资水平、待遇支出及基金结存差异显著，直接后果是医疗保障的收入再分配效应不明显，低收入人群的受益程度低于高收入人群，经济落后地区的筹资负担高于沿海发达地区，居民医保的累计基金结存远低于职工医保。针对这些问题，应通过合理设计筹资费率与转移支付标准、提高医保基金统筹层次、盘活沉淀资金等措施，强化基本医疗保障制度的统筹共济功能，促进不同人群、不同区域的健康公平。
Abstract:
At present, China's basic medical insurance system has certain limitations in the process of progressive reform. The separation of the urban and rural residents' health insurance and urban employees' health insurance systems, superimposed on the imbalance of the health insurance financing mechanism, financing level and central financial transfer payments due to regional division, have together shaped the horizontal inequity of China's basic medical security system. The direct consequence of this phenomenon is that the income redistribution effect of medical insurance is not obvious, with low-income people benefiting to a lesser extent than high-income people, the financing burden of economically backward regions being higher than that of developed coastal regions, and the accumulated fund balance of residents 'medical insurance being much lower than that of employees' medical insurance. In order to solve the problem of unbalanced and insufficient development of medical insurance, it is necessary to reasonably design inter-regional financing rates and transfer payment standards, raise the level of medical insurance fund co-ordination, and further revitalize the huge amount of deposited funds. This will strengthen the coordinated mutual aid function of the basic medical security system, and promote health equity across different populations and regions.
