目的：分析中医药医保政策文本的重点主题及其演变趋势，为未来医保政策研究与制定提供参考。方法：选取2017年至2025年7月共87份中医药医保政策为样本，运用LDA主题模型，识别主题信息，通过主题强度观察各主题的关注度及演变趋势。结果：从国家、省级、市级政策文本中分别获取了386个、1 399个、1 381个关键词；通过LDA主题模型共生成8个主题，进一步归纳为4类；其中，“中医药医保支付方式”的关注度逐年提高，“中医药医保支付范围”“中医药商业保险”的关注度近年有下降趋势，“中医药医保基金监管”的关注度历年差异不大。结论：我国中医药医保政策围绕支付方式、支付范围、基金监管、商业保险4大主题进行制度体系建设，国家级和省级政策的关注点具有较高的一致性，市级政策具有较强的落地性，中医药医保支付方式改革或将成为未来关注重点。
Abstract:
Objective To analyze the key themes and evolutionary trends in the texts of medical insurance policies of traditional Chinese medicine(TCM), and to provide references for future policy research and formulation. Methods A total of 87 TCM medical insurance policy documents from 2017 to July 2025 were selected as samples. The LDA topic model was applied to identify topic information, and the focus and evolutionary trends of each theme were observed through topic intensity. Results A total of 386, 1 399, and 1 381 keywords were extracted from national, provincial, and municipal policy texts, respectively. The LDA topic model generated 8 topics, which were further categorized into 4 groups. Among these, the focus on "TCM medical insurance payment methods" had increased annually, while the focus on "TCM medical insurance payment scope" and "TCM commercial insurance" had shown a declining trend in recent years. The focus on "TCM medical insurance fund supervision" had remained relatively stable over the years. Conclusion China's TCM medical insurance policies focus on constructing an institutional system around four major themes: payment methods, payment scope, fund supervision, and commercial insurance. National and provincial policies show high consistency in their focus areas, while municipal policies demonstrate stronger practicality in implementation. The reform of TCM medical insurance payment methods may become a key focus in the future.
