摘要:
在DRG支付背景下，医院高额费用病例的产生存在多元因素，包括多病共存、病情危重、住院天数超长、新技术新项目应用及过度诊疗等。对此，医院应建立多部门协作、院科两级审核及“质控—反馈—整改”闭环管理体系，实现对费用异常病例的精准识别与分类处置。将特例单议申报融入高额费用监测流程，通过建立规范化反馈、电子台账及多层级复核机制，有效区分合理与不合理的高额费用，既能保障疑难危重患者的合理支付，又能促进诊疗行为与病案首页质量的持续改进，为医院实现精细化管理提供了重要保障。
Abstract:
Under the DRG payment system, the occurrence of high-cost cases in hospitals results from multiple factors, including multi-morbidity, critical illness, prolonged hospitalization, application of new technologies, and irrational medical practices. In response, hospitals should establish a multi-department collaboration mechanism, a hospital-department two-tier review mechanism, and a closed-loop management system of "quality control-feedback-rectification" to achieve precise identification and classified management of abnormal-cost cases. The application for special case separate discussion can be integrated into the high-cost case monitoring process. Through standardized feedback mechanisms, electronic record-keeping, and multi-level review systems, reasonable and unreasonable high costs can be effectively distinguished. This approach not only ensures appropriate reimbursement for critically ill patients with complex conditions but also promotes continuous improvement in medical practices and the quality of medical record front pages, providing important support for refined hospital management.
