基于复杂动态系统理论,运用解释结构模型(ISM)和交叉影响矩阵相乘法模型(MICMAC),分析紧密型县域医共体建设的影响因素及其影响机制。ISM结果显示,紧密型县域医共体建设的影响因素可分为3个层级:表层次因素是基层医疗服务能力,中间层因素包括绩效考核激励、医保打包支付、部门协同合作、信息化建设程度、医护人力资源、专业化部门设立,深层次因素是政策配套支持、政府注意力分配;MICMAC模型进一步验证了ISM的分层结果,将各个影响因素归纳为依赖因素群、自发因素群和独立因素群。据此建议:深化认知共识,构建高位协同的组织领导体系;强化政策耦合,完善多元协同的保障支撑机制;推进数智赋能,打造智慧协同的一体化信息平台。
Abstract:
From the perspective of the theory of complex dynamic systems, this study employed the Interpretive Structure Model(ISM) and the Matrix of Cross Impact Multiplications Applied to Classification(MICMAC) to analyze the influencing factors and influence mechanisms of building compact county-level medical communities. The ISM results indicated building compact county-level medical communities could be categorized into three levels. Primary healthcare service capacity was surface-level factor. Middle-level factors included performance assessment incentives, bundled medical insurance payment, departmental collaboration and cooperation, the degree of informatization construction, medical and nursing human resources, and the establishment of specialized departments. Deep-level factors consisted of the Policy supports and the allocation of government attention. The MICMAC model further verified the stratification results of ISM, categorizing the various influencing factors into dependent, endogenous, and independent factor groups. Based on the findings, the suggestions are proposed to deepen cognitive consensus and build a high-level collaborative organizational leadership system;, strengthen policy coupling and improve a diversely collaborative support mechanism, and promote digital and intelligent empowerment to create a smart and collaborative integrated information platform.
