摘要:
当前，县域医共体的治理困境既包含权力配置失衡引发的体制性障碍，也存在服务协同不足、资源错配、医保激励不足、数智技术应用滞后等机制性问题；基于“控制钮框架”理论，以权力、服务、资源、利益、技术为“控制钮”，为破解县域医共体治理难题、优化体制机制提供了新的思路——以权力结构的契约化重塑为核心，撬动服务模式向健康本位转型，驱动健康资源精准高效配置，并通过医保战略性购买与数智技术赋能，构建激励相容的治理生态，从而推动县域卫生服务体系高质量发展。
Abstract:
Currently, the governance dilemmas of county-level medical communities include both institutional obstacles caused by imbalanced power allocation and mechanistic issues such as insufficient service coordination, resource misallocation, inadequate medical insurance incentives, and lagging digital technology application. Based on the "Control Knob Framework" theory, this study identified power, service, resources, interests, and technology as the "control knobs", providing new insights for resolving governance challenges and optimizing the management systems and mechanisms of county-level medical communities. This approach centers on the contractual reshaping of power structures, leverages the transformation of service models toward a health-oriented paradigm, drives precise and efficient allocation of health resources, and establishes an incentive-compatible governance ecosystem through the strategic purchasing of medical insurance and the empowerment of digital technologies, thereby promoting the high-quality development of county-level health service systems.
