目的：分析湖北省基层中医药服务的现实困境，探索适配中医药行业特点的发展路径。方法：以湖北省20家社区卫生服务中心负责人及一线中医临床医生为调研对象，开展半结构化访谈，运用Nvivo15.0软件进行主题分析。结果：湖北省基层中医药服务面临政策支持与医保适配不足、中医人才短缺与师承传承断裂、医联体中医药专项协同低效、居民认可度有限等现实困境。结论：完善政策体系，包括完善基层中医药人才保障机制、提升中医药文化认同、深化医联体中医药专项协同、加强特色服务与智慧应用，破解发展瓶颈，实现基层中医药服务高质量发展。
Abstract:
Objective To analyze the practical dilemmas of primary-level traditional Chinese medicine (TCM) services in Hubei Province and explore development pathways adapted to the characteristics of the TCM industry. Methods Directors of 20 community health service centers and frontline TCM clinicians in Hubei Province were selected as survey respondents. Semi-structured interviews were conducted, and thematic analysis was performed using Nvivo 15.0 software. Results Primary-level TCM services in Hubei Province faced practical dilemmas including insufficient policy support and medical insurance alignment, shortage of TCM talent and disruption of mentorship transmission, inefficient specialized TCM collaboration within medical alliances, and limited public recognition. Conclusion It is necessary to improve the policy system by refining the talent security mechanism for primary-level TCM, enhancing TCM cultural identity, deepening specialized TCM collaboration within medical alliances, and strengthening distinctive services and smart applications, so as to overcome development bottlenecks and achieve high-quality development of primary-level TCM services.
