目的:系统回顾2020—2025年全球老年长期照护经济学评价研究的进展,为我国老年长期照护政策体系优化提供参考。方法:通过系统文献检索与内容分析,总结老年长期照护的评价工具,比较各国机构照护、社区照护及居家照护模式的成本效果及长期护理保险的筹资模式。结果:单一QALY框架难以全面评估老年人长期照护的多维价值,社会关怀、功能维持和非正式照护者效用已成为新兴补充维度;多国研究表明,居家/社区照护对中轻度失能老人更具成本效益,但其优势依赖于完善的医疗服务衔接与持续的资金支持;社会保险与公私合营筹资机制可有效提升老年长期照护的可负担性。结论:我国需进一步拓展老年长期照护多维价值评价方法,优化筹资结构与支付方式,完善对家庭照护者的支持政策,构建公平、高效、可持续的长期照护服务体系,切实应对人口老龄化带来的巨大挑战。
Abstract:
Objective To systematically review the progress in global economic evaluations of elderly long-term care (LTC) from 2020 to 2025 and provide references for optimizing China's elderly LTC policy system. Methods Through systematic literature retrieval and content analysis, evaluation tools for elderly LTC were summarized, and the cost-effectiveness of institutional care, community care, and home-based care models as well as the financing models of LTC insurance in various countries were compared. Results The traditional QALY framework alone was insufficient to comprehensively assess the multidimensional value of elderly LTC; emerging supplementary dimensions included social care, functional maintenance, and the utility of informal caregivers. Multinational studies indicated that home/community-based care was more cost-effective for the elderly with mild to moderate disabilities, but its advantage relied on well-integrated healthcare services and sustained funding support. Social insurance and public-private partnership financing mechanisms could effectively enhance the affordability of elderly LTC. Conclusion China should further develop multidimensional value evaluation methods for elderly LTC, optimize financing structures and payment methods, improve support policies for family caregivers, and build a fair, efficient, and sustainable LTC service system to effectively address the significant challenges posed by population aging.
