摘要:
运用SWOT方法分析基层医疗机构主导型医养结合模式的比较优势、内部劣势、外在机遇和发展障碍。以基层医疗机构为主导的社区医养结合模式对解决病残老年人、孤寡老年人和中低收入老年人的养老难、就医难问题具有较大优势，可借力社区卫生服务网络构建社区医养结合服务网络，推动基层医疗机构开展社区医养结合服务。
Abstract:
SWOT method is used to analyze the comparative advantages， internal disadvantages， external opportunities and development obstacles of the leading medical-care combination model of primary medical institutions. The community-based combination of medical and health care institutions， which is led by grassroots medical institutions， has great advantages in solving the difficulty of getting care and medical service among the elderly. We can raise the service network and promote grassroots medical institutions to carry out community medical care and combined services.
