目的：比较不同医保类型恶性肿瘤终末期患者卫生服务利用差异。方法：随机抽取某市医保数据库2015—2017年1678名恶性肿瘤终末期医保患者，采用t检验与卡方检验，对不同医保类型患者卫生服务利用情况进行比较分析。结果：职保患者的卫生服务利用高于居保患者，两类患者在医疗机构和治疗方式的选择上存在差异，放化疗是恶性肿瘤终末期患者的主要选择，极少人选择舒缓治疗。结论：制定并实施肿瘤终末期诊疗指南或规范，构建上下联动的舒缓疗护网络，选择合理医疗方式，提高医疗资源利用效率。
Abstract:
Objective To compare the differences in health service utilization among patients with end-stage malignant tumors of different types of medical insurance. Methods A total of 1678 patients with end-stage malignant tumor of different medical insurance from 2015 to 2017 were randomly selected. The t-test and chi-square test were used to compare the health service utilization of patients with different types of medical insurance. Results The utilization of health services for patients with occupational insurance was higher than that of patients with residential care. There were differences in the choice of medical institutions and treatment methods between the two types of patients. Chemotherapy were the main choice for patients with end-stage malignant tumors， and few people chose soothing treatment. Conclusion We should develop and implement guidelines and norms for end-stage cancer diagnosis and treatment， build a network of soothing treatments that link up and down， select reasonable medical methods， and improve the utilization efficiency of medical resources.
