摘要:
现有的慢性病管理模式难以满足患者对慢性病防治的需求。对此，本文构建了基于社群经济的“互联网+”慢性病管理模式，即在患者层面构建以自我管理为导向的“患-患”互助社群，在医生层面构建以知识更新为导向的“医-医”互动社群，通过医患合作，构建以疾病康复为导向的“医-患”共治平台，最终形成“互联网+”慢性病管理产业生态圈。通过对病友互助平台的案例分析，证明该管理模式的可行性，并从政府和企业的角度，提出了实现该管理模式的对策建议。
Abstract:
The current chronic disease management model cannot meet the demands of patients for the prevention and treatment of chronic diseases. In this regard, this article constructs a "Internet +" chronic disease management model based on the community economy, that means to build a self-management-oriented "patient to patient" mutual assistance community at the patient level, and a knowledge update-oriented "doctor to doctor" mutual assistance community at the doctor level. Then build a "medicine-patient" co-governance platform with disease rehabilitation oriented by cooperation between doctors and patients, and finally forms the "Internet +" chronic disease management industry ecosystem. Through the case analysis of the patient mutual assistance platform, try to prove the feasibility of the management model, and put forward countermeasures and suggestions for realizing the management model from the perspective of government and companies.
