摘要:
目的：分析DRG付费改革对医院运营效率的影响。方法：以浙江省某三甲综合医院为研究样本，从医疗服务能力、医疗服务效率、医疗质量与安全、医保监管4个维度，采用Wilcoxon秩和检验、χ2检验或Fisher精确检验方法，分析DRG付费改革对医院运营效率的影响。结果：DRG付费改革后，CMI值、DRG组数、病组覆盖率增加，四级手术、微创手术、日间手术占比提高；住院天数和住院费用显著降低，时间消耗指数和费用消耗指数下降；同时，医疗质量与安全、医保监管水平未因改革而下降。结论：浙江省DRG付费改革初见成效，有助于提高医院医疗服务能力和医疗服务效率，并兼顾医疗质量与安全，强化医保监管意识。
Abstract:
Objective To analyze the influence of DRG payment reform on hospital operation efficiency. Methods Taking a 3A general hospital in Zhejiang Province as the sample, from the four dimensions of medical service capacity, medical service efficiency, medical quality and security, and medical insurance supervision, Wilcoxon rank sum test, χ2 test or Fisher exact test were used to analyze the influence of DRG payment reform on hospital operation efficiency. Results After the DRG payment reform, the CMI value, the number of DRG groups and the coverage rate of the disease group increased; the proportions of grade IV surgery, minimally invasive surgery and pediatric day surgery increased; the hospitalization days and expenses decreased significantly; and the time consumption index and cost consumption index decreased significantly. At the same time, the quality and security of medical care and the level of medical insurance supervision did not decrease as a result of the reform. Conclusion DRG payment reform in Zhejiang Province has achieved initial results, which is helpful to improve the medical service capacity and efficiency of hospitals, give consideration to medical quality and security, and strengthen the consciousness of medical insurance supervision.
