摘要:
在慢性病签约管理的准备、签约、诊疗、用药、监测预警、随访、健康教育、转诊和履约管理等各个环节，信息化均发挥了有效的助力作用。信息化提升了家庭医生团队的慢性病综合干预能力，改善了慢性病签约患者的获得感。因此，需持续推进慢性病相关信息互通共享和数据分析使用，发挥信息化在构建医患互信、全专互动、三医联动等关系中的支撑作用，健全信息安全和医疗责任分担机制，从而进一步助力慢性病签约管理。
Abstract:
Informatization has played an effective role in the preparation, contracting, diagnosis and treatment, medication, monitoring and early warning, follow-up, health education, referral and contracting performance management of chronic disease contracting management. Informatization has improved the comprehensive intervention capability of the family doctor team and improved the sense of gain of contracted patients. Therefore, it is necessary to consistently promote the exchange and sharing of chronic disease-related information and the use of data analysis, improve the supporting role of informatization in the establishment of doctor-patient mutual trust, specialist-primary care physician interaction, and medication-drugs-insurance linkage. It is suggested to improve information security and medical liability sharing mechanisms, so as to further help chronic disease contracting management.
