目的：分析中老年糖尿病患者直接医疗负担的公平性。方法：使用2011年和2018年CHARLS问卷中的中老年糖尿病患者门诊和住院数据，测算直接医疗负担，利用集中指数分析不同患者群体直接医疗负担的公平性。结果：2018年中老年糖尿病患者的患病率（12.34%）较2011年（5.86%）大幅升高；2011和2018年的糖尿病患者人均直接医疗负担分别为7 848元和9 869元，个人支出分别为5 022元和5 753元；65岁以上、初中学历以下、高收入、城镇患者群体的直接医疗负担更高，集中指数由2011年的0.354下降到2018年的0.257。结论：中老年糖尿病患者的直接医疗负担较重，其中高收入群体直接医疗负担最重，需从政府、医疗机构、居民个人三方面有效协同，共同抵御糖尿病疾病经济风险，最大程度减轻慢性病直接医疗负担。
Abstract:
Objective To analyze the fairness of direct medical burden of middle-aged elderly with diabetes. Methods Using the outpatient and hospitalization data of middle-aged elderly with diabetes from the 2011 and 2018 wave of CHARLS, the direct medical burden was measured, and the fairness of the direct medical burden in different patient groups was analyzed using the centralization index. Results The prevalence of middle-aged elderly with diabetes in 2018 (12.34%) was significantly higher than that in 2011 (5.86%). The per capita direct medical burden of diabetic patients in 2011 and 2018 was 7 848 yuan and 9 869 yuan respectively, meanwhile the personal expense was 5 022 yuan and 5 753 yuan respectively. The direct medical burden was higher among the elderly aged over 65, with educational achievement below junior high school education, with high income, and living in urban areas. The concentration index has dropped from 0.354 in 2011 to 0.257 in 2018. Conclusion The direct medical burden of middle-aged elderly with diabetes is relatively heavy, and it is necessary to coordinate effectively from the government, medical institutions, and individual residents to jointly resist the economic risk of diabetic diseases and reduce the direct medical burden of chronic diseases to the greatest extent.
