目的：从规范医疗行为的视角，探讨医疗服务价格结构调整策略。方法：以湖北省某三甲公立医院为例，分析医疗服务价格改革对医院医疗收入和患者医疗费用的影响。结果：取消药品加成和耗材加成后，医院整体弥补率为72.38%和57.43%，低于预期水平；价格调整后，综合医疗服务类、临床诊疗类和中医及民族医疗类项目收入调增，医技诊疗服务类项目收入调减，患者次均检查和化验费用占比增长较快。结论：按照“有保有压、总量控制”的医疗服务价格结构调整策略，建立“两降低”“两提高”“一增加”的价格结构动态调整机制，实现医疗服务的公平与效率。
Abstract:
Objective To explore strategies for adjusting medical service price structure from the perspective of standardizing medical behavior. Methods Taking a tertiary public hospital in Hubei province as an example, the impact of medical service price reform on hospital medical income and patient medical expenses was analyzed. Results After canceling the drug markup and consumable markup, the overall compensation rates were 72.38% and 57.43% in the hospital, which were lower than expected levels. After the price adjustment, the income of comprehensive medical services, clinical diagnosis and treatment, and traditional Chinese medicine and ethnic medicine projects increased, while the income of medical technology diagnosis and treatment services projects decreased. The proportion of average examination and laboratory expenses per patient has increased rapidly. Conclusion According to the strategy of adjusting the price structure of medical services based on the principle of " maintaining in some areas while reducing in others, controlling the total quantity", it is necessary to establish a dynamic adjustment mechanism for the price structure of "two reductions" "two improvements" "one increase", in order to achieve fairness and efficiency in medical services.
