目的：探讨我国各省份医疗卫生资源的多项指标在时空分布上的不公平性及其影响因素。方法：选取2009—2022年卫生机构数、床位数和卫生人员数，通过熵权法计算综合评价得分，利用空间基尼系数和新兴时空热点分析评价资源配置的时空公平性，运用时空地理加权回归模型识别资源配置的驱动因素及其时空异质性影响。结果：我国医疗卫生资源呈现“东高西低”的地理格局，四项指标均存在显著的地区不公平性，且热点区域集中在东部地区；经济发展水平、人口结构等因素对医疗卫生资源的影响具有显著的空间差异。结论：我国医疗卫生资源总量持续增长，但地区间资源配置及其驱动因素仍存在显著差异；政府在制定相关政策时应因地制宜，重点优化西部地区的资源配置，东南地区应加快老龄化医疗卫生资源配置。
Abstract:
Objective To explore the spatiotemporal inequity and their influencing factors of multiple indicators of healthcare resources across provinces in China. Methods Using data from 2009 to 2022, three core indicators, namely, the number of healthcare institutions, hospital beds, and healthcare personnel, were selected, and the entropy weight method was applied to assess the composite score. The spatial Gini coefficient and the emerging spatiotemporal hotspot analysis were used to evaluate the spatiotemporal equity of healthcare resource allocation. The geographically and temporally weighted regression model was employed to identify the drivers of healthcare resource allocation and their effect of spatiotemporal heterogeneous. Results The allocation of healthcare resources showed a geographical pattern of "high in the east and low in the west" in China, with significant regional inequity in all four indicators, and high-resource hotspots were predominantly located in the eastern region. Economic development levels and population structure exhibited significant spatial heterogeneity in their impact on healthcare resources. Conclusion The government should make relevant policies according to local conditions, focusing on optimizing the allocation of healthcare resources in the western region, and strengthen the allocation of healthcare resources for the aging in the southeastern region.
