深化以公益性为导向的公立医院改革是当前医改的重要任务，健全的财政投入机制有助于更好地实现公立医院公益性。文章从患者角度界定公立医院公益性的内涵，结合公立医院的事业单位属性，对公立医院财政投入与公益性的关系进行分析。研究发现，2011—2024年，财政对公立医院的投入从4 039.05亿元增长至21 559.58亿元，并坚持供需兼补的方式，为公立医院实现公益性提供了有力支撑；但公立医院持续超规划发展，超出了现有财力和医保筹资能力的承受范围；公立医院运行机制改革滞后，加剧了完善财政投入机制的难度；公立医院财政投入方式转型不彻底，财政投入机制有待进一步优化。据此提出建议：完善公立医院治理机制，促进公立医院高质量发展；严格控制大型公立医院发展规模，使其与财力和医保筹资能力相适应；优化公立医院财政投入机制，进一步彰显公立医院公益性。
Abstract:
Deepening the reform of public hospitals towards public welfare is a crucial task in the current healthcare reform, and a robust fiscal investment mechanism contributes to the better realization of the public welfare nature of public hospitals. This study defines the connotation of public welfare from the perspective of patients, and in conjunction with the public institution attributes of public hospitals, analyzes the relationship between fiscal investment and the public welfare nature. The findings indicate that from 2011 to 2024, fiscal investment in public hospitals increased from 403.905 billion yuan to 2 155.958 billion yuan, providing strong support for their public welfare function through a dual-supply-and-demand subsidy approach. However, the sustained development of public hospitals beyond regional health planning has exceeded the capacity of existing fiscal resources and medical insurance financing. The lagging reform of their operational mechanisms has exacerbated the difficulty in improving the fiscal investment mechanism, and the incomplete transition in fiscal investment methods suggests that the mechanism still requires further optimization. Accordingly, recommendations are proposed: improve the governance mechanism of public hospitals to promote high-quality development; strictly control the development scale of large public hospitals to align them with fiscal and medical insurance financing capacities; and optimize the fiscal investment mechanism of public hospitals to further demonstrate their public welfare nature.
