摘要:
社会处方通过将患者与社区非医疗资源相连接，为慢性病管理提供了新思路。国际经验表明，社会处方有助于改善健康结果、促进健康公平性、缓解基层医疗压力。我国在实施社会处方过程中，面临政策制度、资源整合、专业队伍等方面的多重挑战。对此，可借鉴国际经验，基于社区慢性病管理实际，构建社会处方的标准化转介体系，建立多元化融资机制，加强多学科能力建设，形成动态评估反馈框架，从而助力实现“健康中国”战略目标。
Abstract:
Social prescribing provides a new approach for chronic disease management by connecting patients with community-based non-medical resources. International experience shows that social prescribing helps improve health outcomes, promote health equity, and alleviate pressure on primary healthcare. In the process of implementing social prescribing, China faces multiple challenges in terms of policy systems, resource integration, and professional workforce development. In response, drawing on international experience and based on the reality of community chronic disease management, it is advisable to establish a standardized referral system for social prescribing, develop a diversified financing mechanism, strengthen multidisciplinary capacity building, and form a dynamic evaluation and feedback framework, thereby contributing to the achievement of the "Healthy China" strategic goals.
