摘要:
　目的：分析县域医共体慢病一体化管理的实施困境，提出优化策略。方法：采用分层随机抽样法对安徽省6家医共体818名在岗人员进行问卷调查，并对6名关键知情人进行质性访谈。结果：医共体慢病一体化管理在制度执行、组织协同、专业人才配置、转诊服务衔接以及主体职责落实方面存在明显阻碍。结论：医共体慢病一体化管理需要进一步强化制度协同，破除协作壁垒，加强人才队伍建设，优化转诊机制，提升居民健康素养，从而实现高效协同运行。
Abstract:
Objective To analyze the implementation dilemmas of integrated management of chronic diseases in county-level medical communities, and to propose optimization strategies. Methods A stratified random sampling method was used to conduct a questionnaire survey of 818 on-duty staff members from six county-level medical communities in Anhui Province, and qualitative interviews were conducted with six key informants. Results There were obvious obstacles in the implementation of integrated management of chronic diseases in county-level medical communities, such as institutional implementation, organizational coordination, allocation of specialized personnel, coordination of referral services, and the implementation of responsibilities by various medical entities. Conclusion To achieve efficient and coordinated operation of integrated management of chronic diseases in county-level medical communities, it is necessary to strengthen institutional coordination, break down barriers to collaboration, enhance workforce development, optimize referral mechanisms, and improve residents' health literacy.
