目的：科学测度山东省公立医院高质量发展与公益性水平，分析两者的耦合协同关系及其障碍，为深化公立医院改革提供实证参考。方法：基于2017—2023年山东省39家公立医院数据，运用EBM—超效率模型、熵权法、耦合协调度模型及“失调度”指标，分析两大系统的耦合协同关系及其障碍。结果：2017—2023年，山东省39家公立医院高质量发展水平显著提升（均值从0.702升至0.856），增幅达21.96%；同期公益性水平也稳步提升，增幅9.85%；两者耦合协调度均值从0.760提升至0.821，由“中度协调”转为“高度协调”；“失调度”分析显示，“高质量发展—服务可及”维度失调度均值为0.289 3，是协同发展的核心短板，“高质量发展—收入均衡”维度失调度均值为0.208 3，反映医院对住院收入的路径依赖未扭转；“高质量发展—费用合理”维度失调度均值为0.153 5，体现医保支付方式改革等政策控费成效显著。结论：山东省公立医院高质量发展与公益性的协同水平有所提升，但仍面临服务可及性不足与收入结构失衡等协同障碍；建议实施分层分类引导政策，构建以公益性为导向的绩效评价体系，强化医疗、医保、医药政策协同，促进公立医院高质量发展与公益性水平的有效协同。
Abstract:
Objective To scientifically measure the high-quality development and public welfare levels of public hospitals in Shandong Province, evaluate the coupling synergy between the two and the barriers to this relationship, and to provide empirical references for deepening public hospital reform. Methods Based on panel data from 39 public hospitals in Shandong Province from 2017 to 2023, the EBM super-efficiency model was used to measure the level of high-quality development, while the entropy weight method was employed to assess public welfare levels. Coupling coordination degree models and "lack of coordination" indicators were applied to analyze the coupling synergy between the two systems and dimensional barriers. Results During the study period, the level of high-quality development in the 39 public hospitals in Shandong Province improved significantly(with an average annual growth rate of 3.36%), representing a 21.96% increase. The level of public welfare also rose steadily, with an increase of 9.85%. The mean coupling coordination index between the two systems rose from 0.760 to 0.821, shifting from "moderate coordination" to "high coordination". The "lack of coordination" analysis revealed that the average score for the "high-quality development - service accessibility" dimension was 0.289 3, representing the primary barrier to synergy. The average score of "lack of coordination" indicators for the "high-quality development - income balance" dimension was 0.208 3, reflecting that the hospital's path dependence on inpatient revenue had not been reversed. The average score of "lack of coordination" indicators for the "high-quality development - cost rationality" dimension was 0.153 5, demonstrating the significant effectiveness of cost-control policies such as medical insurance payment method reform. Conclusion The synergy between high-quality development and public welfare in public hospitals in Shandong Province has improved but still faces structural challenges such as insufficient service accessibility and imbalanced income structures. It is recommended to implement tiered and categorized guidance policies, construct a public welfare-oriented performance evaluation system, and strengthen policy coordination among medical services, health insurance, and pharmaceutical systems, so as to promote effective synergy between high-quality development and public welfare in public hospitals.
