目的：探索和设计医用耗材集中带量采购背景下按病种分值付费（DIP）优化方案。方法：以广东省G市2017—2020年5个涉及冠状动脉支架使用的病组为例，设计医用耗材集中带量采购后DIP优化方案。结果：理论上，冠状动脉支架集中带量采购后，患者住院费用和冠状动脉支架费用平均降低18 081.51元和23 974.33元；DIP优化方案是将总分值拆分为冠状动脉支架分值和其他分值，5个病组的总分值分别下降了822、1 609、2 813、2 078、2 008。结论：集中带量采购背景下，医用耗材分值单独测算是DRG/DIP的主要优化方向，但仍须遵循“打包”思想，同时加快医保编码数据库建设，提高病案首页数据质量。
Abstract:
Objective To explore and design the optimization scheme of per-patient-divisional payment (DIP) in the context of centralized quantity procurement of medical consumables. Methods Five patient groups involving the use of coronary stents in G city, Guangdong province, from 2017 to 2020 were used as examples to design DIP optimization schemes after centralized banded procurement of medical consumables. Results Theoretically, the average reduction in patient hospitalization cost and coronary stent cost was RMB 18 081.51 and RMB 23 974.33 after centralized banded procurement of coronary stents. The DIP optimization scheme was to split the total score into coronary stent score and other scores, and the total scores of the five patient groups decreased by 822, 1 609, 2 813, 2 078 and 2 008 respectively. Conclusion Under the background of centralized quantity procurement, the separate measurement of medical consumables score is the main direction of DRG/DIP optimization, but still must follow the idea of "packaging". And at the same time, it is necessary to accelerate the construction of medical insurance coding database to improve the quality of the first page data of medical cases.
