目的：分析北京市耗材集采联合DRG付费扩面的实施效果。方法：基于2022—2024年北京市二级及以上医院出院数据，以骨脊柱融合手术DRG组为研究对象，采用两组间断时间序列分析等方法，分析联合政策对住院医疗服务绩效的影响。结果：联合政策实施后，实验组死亡率从0.088%降至0.063%，月均出院人数从2 093人增至2 187人，例均住院费用从114 474.46元降至83 532.41元；对照组的例均住院费用无明显变化，两组间即刻改变量差异有显著性意义。结论：北京市耗材集采联合DRG付费扩面产生了费用水平及其变化趋势“双下降”的协同效应，改善了住院医疗服务绩效；后续要加强长期医疗质量监测，并以DRG病种负担为导向，积极稳妥地推进耗材集采扩面。
Abstract:
Objective To analyze the implementation effect of the expansion of volume-based procurement of consumables combined with DRG payment in Beijing. Methods Based on discharge data from secondary and tertiary hospitals in Beijing from 2022 to 2024, spinal fusion surgery DRG groups were selected as the study subjects. A two-group interrupted time series analysis and other methods were used to assess the impact of the combined policy on inpatient healthcare service performance. Results After the implementation of the combined policy, the mortality rate in the intervention group decreased from 0.088% to 0.063%, the average monthly number of discharges increased from 2 093 to 2 187, and the average cost per case decreased from 114 474.46 yuan to 83 532.41 yuan. The average cost per case in the control group showed no significant change, and the immediate change difference between the two groups was statistically significant. Conclusion The expansion of volume-based procurement of consumables combined with DRG payment in Beijing has generated a synergistic effect, leading to a "dual reduction" in both the level and growth trend of costs, thereby improving inpatient healthcare service performance. It is recommended to strengthen long-term medical quality monitoring and actively yet prudently promote the expansion of volume-based procurement of consumables, guided by the burden of DRG disease categories.
