目的：探究医养结合服务可持续发展的路径，为促进医养结合服务供需匹配与效能提升提供理论依据和实践指导。方法：基于社会生态系统理论框架，运用模糊集定性比较分析方法，探讨医养结合服务高可持续性的组态路径。结果：医养结合服务高可持续性的组态路径主要包括长期护理保险制度引领型、基本医疗保险制度引领型和供需匹配型三种类型，区域人口老龄化程度、区域老年人口需求评估、核心人力资源配置、能力建设、监管政策体系建设及社会保障资金支持作为核心条件深刻影响医养结合服务的可持续发展。结论：从优化顶层制度设计、强化政策支持，促进基层服务能力建设、构建全要素服务网络，精准对接需求、促进供需匹配三个层面系统施策，实现医养结合服务的可持续发展。
Abstract:
Objective To explore pathways for the sustainable development of integrated medical and elderly care services, and to provide a theoretical basis and practical guidance for promoting supply-demand matching and enhancing service effectiveness. Methods Based on the social-ecological systems theory framework, fuzzy-set qualitative comparative analysis(fsQCA) was used to explore the configurational pathways leading to high sustainability of integrated medical and elderly care services. Results The configurational pathways for high sustainability mainly include three types: the long-term care insurance system-led type, the basic medical insurance system-led type, and the supply-demand matching type. Core conditions such as the degree of regional population aging, assessment of regional elderly needs, allocation of core human resources, capacity building, development of regulatory policy systems, and social security funding profoundly influence the sustainable development of integrated services. Conclusion To achieve sustainable development of integrated medical and elderly care services, systematic measures should be implemented across three dimensions: optimizing top-level institutional design and strengthening policy support; promoting grassroots service capacity building and constructing an all-element service network; and precisely matching demand to promote supply-demand alignment.
