目的：分析上海市老年人治疗费用，为控制老年人治疗费用和优化筹资结构提供依据。方法：基于“SHA2011”核算方法，从“功能-机构-筹资”三个维度，对比分析上海市老年人治疗性卫生费用。结果：2014—2018年，上海市老年人治疗费用年均增长7.80%；老年人治疗费用具有病种聚集特征，四种疾病治疗费用增长41.34%；老年人治疗费用的主要筹资来源是公共筹资，2018年公共筹资589.09亿元，占77.68%，比2014年上升13.15百分点。结论：老龄化加重了老年人治疗费用负担，应加强重点病种管理，积极推进医养护结合，进一步强化分级诊疗制度实施效果和优化治疗费用筹资结构。
Abstract:
Objective To analyze the cost of treatment for the elderly in Shanghai, and to provide evidence for controlling the cost of treatment and optimizing the financing structure. Methods Based on the "SHA2011" accounting method, this study analyzed the therapeutic health expenses of the elderly in Shanghai from the three dimensions of "function-institution-funding". Results From 2014 to 2018, the average annual growth rate of the elderly treatment costs in Shanghai was 7.80%. The elderly treatment costs were characterized by clustering of diseases, and four diseases treatment costs increased by 41.34%. The main source of financing for the elderly treatment costs was public financing. In 2018, public financing was 58.909 billion yuan, accounting for 77.68%. It showed an increase of 13.15% over 2014. Conclusion Aging increases the burden of treatment costs for the elderly. It is necessary to strengthen the management of key diseases, actively promote the combination of medical, lont-term, and nursing care, further strengthen the implementation of hierarchical medical system, and optimize the financing structure of treatment costs.
