摘要:
目的：探究儿科病种成本收益现状及临床路径优化对病种成本收益的影响。方法：通过问卷调查，分析三级医院儿科病种成本核算现状，建立病种成本核算体系，对样本医院临床路径优化前后病种成本收益情况进行比较分析。结果：儿科亏损病种多，亏损程度高，劳务性项目亏损严重，临床路径优化能够降低病种例均费用、成本和亏损，实现降本增效。结论：医院应从自身实际出发，基于临床路径建立体现学科特色的病种诊疗管理和成本收益管理体系，鼓励临床科室积极参与临床路径优化实践。
Abstract:
Objective To explore the current situation of cost and benefit of pediatric diseases and the impact of clinical pathway optimization on cost and benefit of diseases. Methods Through the questionnaire survey, the current situation of pediatric disease cost accounting in tertiary hospitals was analyzed, the disease cost accounting system was established, and the cost and benefit of disease before and after clinical pathway optimization in sample hospitals were compared. Results The number of loss-making diseases in pediatrics was large, the degree of loss was high, and the loss of labor related projects was more severe. The optimization of clinical pathway could reduce the average expenses, cost and loss of diseases, and achieve cost reduction and efficiency increase. Conclusion The hospitals should establish disease diagnosis and treatment management and cost-benefit management system that reflect the discipline characteristics based on their own reality and clinical pathway, and encourage clinical departments to actively participate in the optimization practice of clinical pathway.
