摘要:
目的：测算分析新疆农村四类主要慢病的疾病经济负担。方法：采用多阶段分层整群抽样方法，对新疆6个县区11 532名农村常住居民进行问卷调查，采用二步模型法和人力资本法测算四类主要慢病的疾病经济负担，采用多元线性回归分析门诊及住院费用影响因素。结果：2021年新疆农村四类主要慢病的患病率为16.29%；样本人群四类主要慢病的直接经济负担4.21亿元，间接经济负担0.34亿元，心脑血管疾病、恶性肿瘤、糖尿病、呼吸系统疾病经济负担分别为2.89亿元（63.53%）、0.88亿元（19.44%）、0.66亿元（14.47%）、0.12亿元（2.56%）。结论：新疆农村四类主要慢病的疾病经济负担比预想的低，可能受疫情防控的影响，以及与新疆农村地广人稀的特点有关。后疫情时代，应提高基层慢病早期筛查与管理水平,合理分级诊疗，缩短平均住院天数，切实减轻慢病患者的疾病经济负担。
Abstract:
Objective To measure and analyze the economic burden of four major chronic diseases in rural areas of Xinjiang. Methods A multi-stage stratified cluster sampling method was used to conduct a questionnaire survey on 11 532 rural residents in six counties and districts of Xinjiang, and a two-step model method and a human capital method were used to measure the economic burden of four major chronic diseases, and a multivariate linear regression method was used to analyze the factors influencing the costs of outpatient and inpatient. Results The prevalence rate of four major chronic diseases was 16.29% in rural areas of Xinjiang in 2021. The direct economic burden of four major chronic diseases in the sample population was 421 million yuan, and the indirect economic burden was 34 million yuan. The economic burden of cardiovascular and cerebrovascular diseases, malignant tumors, diabetes mellitus, and respiratory diseases was 289 million yuan(63.53%), 88 million yuan(19.44%), 66 million yuan(14.47%), and 12 million yuan(2.56%), respectively. Conclusion The economic burden of four major chronic diseases in rural areas of Xinjiang was lower than expected, possibly influenced by COVID-19 epidemic prevention and control, as well as related to the characteristics of a vast territory with a sparse population in rural areas of Xinjiang. In the post-epidemic era, it is necessary improve the early screening and management of chronic diseases in primary health care institutions, rationalize the hierarchical diagnosis and treatment, and shorten the average hospitalization days so as to effectively reduce the economic burden of chronic diseases.
