摘要:
目的：梳理我国典型地区紧密型县域医共体医保支付方式改革现状，提出完善建议。方法：基于文献研究，以14个典型地区紧密型县域医共体为研究对象，总结其医保支付方式的付费、测算、分配、结余方式以及绩效考核指标现状。结果：14个样本地区的紧密型县域医共体医保基金支付以总额预付为主，总额测算方式主要包括按人头标准测算、根据上年度医保基金支出确定、根据当期筹资规模测算三种，各地根据实际情况进行了改革与创新。结论：紧密型县域医共体医保支付方式改革中，总额预付是前提，医保基金分配和结余留用是关键，绩效考核是保障，同时应重视门诊支付方式改革。
Abstract:
Objective To summarize the current status of medical insurance payment reform in compact county-level medical communities in typical regions of China, and to propose recommendations for improvement. Methods Based on literature review, 14 compact county-level medical communities in typical regions were selected as research subjects, and the current status of payment, calculation, allocation, surplus, and performance evaluation indicators of medical insurance payment methods were summarized. Results In the 14 sample regions, the payment of medical insurance fund in compact county-level medical communities was mainly based on global budget. The total amount calculation methods mainly included three types of calculation based on per capita standards, determination based on the expenditure of the medical insurance fund in the previous year, and calculation based on the current fundraising scale, and Reforms and regions had carried out reforms and innovations according to the actual situation. Conclusion In the reform of medical insurance payment methods in compact county-level medical communities, global budget is the prerequisite, medical insurance fund distribution and the retention of surplus are the key, performance evaluation is the guarantee, and the reforms of outpatient payment methods should be prioritized.
