摘要:
基于2011—2018年三明市某县级公立医院改革的阶段性评估数据资料，运用“状态—结构—绩效”范式，从医疗服务产品属性、改革目标、管理模式、运行机制、监督机制、利益博弈、改革绩效等方面分析其改革过程与效果。分析发现，医疗服务产品具有准公共物品属性，对社会经济发展具有正外部性；县级公立医院改革的整体绩效与制度设计初衷保持一致，可有效缓解 “看病难”问题，以医疗费用、医保基金、医疗收入和职工收入为判定标准的改革效果初显，同时建立了一套较成熟的运行管理机制。在此基础上，为县域医改提出政策建议。
Abstract:
Based on the phased evaluation data of a county-level public hospital reform in Sanming City from 2011 to 2018，using the “state—structure—performance” paradigm，this paper analyzed its reform process and effects from the medical service product attributes，reform goals，management models，operational mechanisms，supervision mechanisms，interest games，and reform performance. The analysis found that medical service products have quasi-public goods attributes and positive externalities for social and economic development；the overall performance of county-level public hospital reforms was consistent with the original intention of system design，which can effectively alleviate the problem of “difficult to see a doctor”；the reform began to take effects using medical expenses，medical insurance，medical income，and employee income as the judgment standard，and a more mature operation management mechanism has been established. On this basis，this paper put forward policy recommendations for county medical reform.
