摘要:
歧义病例是DRG分组路径较为常见的一种变异，其成因包括临床诊疗的复杂性、主要诊断及主要手术选择原则理解差异、转科诊疗信息不对称、分组方案的内生局限性和版本差异、医疗主体的道德风险、科室及手术的特殊性等方面，可能对医保基金结付、医院考核、医疗服务质量、主要诊断及手术选择原则等产生不良影响。对此，应从完善分组方案、引入智能化审核系统、构建多层次质控体系、加强合理入组宣传培训等方面持续优化。
Abstract:
QY cases are a common variation in DRG grouping pathways, caused by factors such as the complexity of clinical diagnosis and treatment, differences in understanding main diagnosis and main surgical selection principles, information asymmetry in transferring between departments, inherent limitations and version differences in grouping schemes, moral hazards of medical subjects, and the specificity of departments and surgeries. QY cases may have adverse effects on medical insurance fund settlement, hospital assessment, medical service quality, main diagnosis and surgical selection principles. In this regard, it is necessary to be continuously optimized from the aspects of improving grouping schemes, introducing intelligent audit systems, building a multi-level quality control system, and strengthening publicity and training on reasonable enrollment.
