目的:探讨公立医院绩效管理失灵的深层原因,提出绩效管理范式重构的路径,以期破解当前医院运营困境。方法:运用社会技术系统理论,从任务、结构、人员、技术四个维度重构绩效管理范式。结果:绩效困境是组织目标错位、结构梗阻、资源错配、文化惯性等系统性问题的外在表现,而非单纯的激励不足;绩效管理作为组织目标的解码系统、资源的配置系统、行为的导向系统,可通过专科任务布局、组织结构变革、人员共识激发、数据赋能技术,实现社会系统与技术系统的联合优化。结论:医院绩效管理需超越“工具”层面,进行系统性、整体性重构,通过四个维度的协同变革,推动医院从规模扩张向价值医疗转型,最终实现可持续发展。
Abstract:
Objective To explore the underlying causes of performance management failures in public hospitals and propose a path for paradigm reconstruction, aiming to address current operational challenges in hospitals. Methods Using sociotechnical systems theory, the performance management paradigm was reconstructed from four dimensions: task, structure, people, and technology. Results Performance challenges were found to be external manifestations of systemic issues such as misaligned organizational goals, structural inefficiencies, resource misallocation, and cultural inertia, rather than being merely a matter of insufficient incentives. As a system for decoding organizational goals, allocating resources, and guiding behaviors, performance management was shown to achieve joint optimization of social and technical systems through specialty task planning, organizational structural reform, staff consensus building, and data-enabled technology. Conclusion Hospital performance management needs to transcend the "tool" level and undergo systematic, holistic reconstruction. Through coordinated changes across the four dimensions, hospitals can transition from scale expansion to value-based healthcare, ultimately achieving sustainable development.
